Agenda:

1.        To review the meeting memo of Apr. 8.

2.        To review a questionnaire toward main survey

 

Discussion: Review and revision of a questionnaire

Based on the yesterday discussion (see meeting minutes0408.doc) and meeting between Japanese staffs after that discussion, a questionnaire was revised and newly typed by Masayo. We discussed about this revision this morning.　 As a result of this meeting, revisions are as follows;

2) Grade of school à What class/year are you in?

5) Are you happy with your present height? à Do you feel happy with your present height? (Because a child who chose "no" in the previous question may not answer the question on the basis of "present" height.)

7) Same as 5). It is going to be "Do you feel happy with your present weight?"

10) Options are going to be "8pm 9pm 10pm 11pm 12midnight".

12) Same as 10).

16) Options are going to be "1. once a day 　2. twice a day　 3. three times a day　 4. others

17) is going to be "How often do you have breakfast?" (Because the options are frequency)

18) Options are going to be made such as "5am 6am 7am Others(　　　　　 )".

20) Options are going to be made such as "10am 11am 12pm 1pm Others(　　　　　 )".

22) Options are going to be made such as "5pm 6pm 7pm Others(　　　　　 )".

25) "Never" is going to be added in options.

26) Same as 25)

27) Options are going to be "1. yes　 2. so-so　 3. no".

32) Bracket is going to be wider.

33) Options are going to be "1. once　 2. twice　 3. three times 　4. rarely 　5. never".

35) In order to be more open-ended, question and options are going to be "When do you wash your hands?　 1. before eating 　2. after eating　 3. after toilet　 4. from outside　 5. others".

36) In order to express more appropriate, question is going to be "How often do you open your bowels?".

39) "Never" is going to be added in options.

40) "Never" is going to be added in options. In order to accord question with options, question is going to be "How often do you get tired?".
